Tibial Cortex Transverse Transport (TTT) Featured in Foot & Ankle International

We are pleased to highlight the recent publication of Tibial Cortex Transverse Transport (TTT) in Foot & Ankle International, the official journal of the American Orthopaedic Foot & Ankle Society (AOFAS).

The article presents a systematic review of 13 clinical studies involving 924 patients treated for ischemic and diabetic foot ulcers, with the majority of cases involving patients with diabetes-related complications.

Understanding Tibial Cortex Transverse Transport (TTT)

Tibial Cortex Transverse Transport (TTT) is an Ilizarov-based limb preservation technique designed to address complex lower limb ischemia and chronic wound healing challenges.

The procedure involves the controlled transverse transport of a segment of the tibial cortex. This mechanical stimulation is proposed to activate biological responses that enhance angiogenesis, improve local microcirculation, and support tissue regeneration.

In clinical practice, TTT is being explored as a potential adjunctive technique in the management of:

  • Chronic limb-threatening ischemia (CLTI)
  • Diabetic foot ulcers
  • Ischemic non-healing wounds

Clinical Findings from the Systematic Review

Across the studies included in the review, TTT demonstrated several consistent clinical trends, including:

  • Improved ulcer healing rates
  • Reduced time to wound closure
  • Improvements in perfusion-related parameters, including ABI
  • Evidence of increased angiogenic activity and microvascular response

The majority of patients included in the analysis were diabetic, highlighting the relevance of TTT in the management of diabetic foot disease and its associated complications.

Clinical Significance and Evolving Evidence

TTT continues to generate significant interest within both orthopaedic and vascular surgical communities as part of a multidisciplinary approach to limb preservation.

The findings of this systematic review suggest that TTT may play a valuable role as an adjunct in complex limb salvage pathways, particularly in patients where traditional revascularisation or wound management strategies are limited.

However, the authors also emphasise that the current evidence base remains in an early stage of development. Most available studies are small, heterogeneous, and observational in nature. As such, there is a clear need for larger, high-quality comparative trials to better define the role of TTT in clinical practice.

Multidisciplinary Collaboration in Limb Preservation

One of the key themes emerging from the literature is the importance of collaboration between orthopaedic and vascular surgical teams in managing complex limb-threatening conditions.

TTT sits at the intersection of these specialties, combining mechanical orthopaedic principles with vascular and regenerative goals. This reinforces the growing importance of multidisciplinary care in achieving improved patient outcomes in limb salvage.
